For patients with severe peripheral artery disease (PAD) who also suffer from chronic kidney disease or diabetes-related kidney impairment, traditional revascularization procedures that rely on iodinated contrast dye can pose a serious risk of further kidney damage. Pedal access zero contrast revascularization eliminates that risk entirely. This advanced technique accesses the arterial system through a tiny puncture in the foot — the pedal artery — rather than the groin, and uses intravascular ultrasound (IVUS) and carbon dioxide (CO2) angiography instead of traditional contrast dye to visualize and treat the blockages. This approach is particularly critical for patients facing critical limb ischemia (CLI) — the most advanced stage of PAD — where blood flow to the foot is so severely compromised that tissue loss, non-healing wounds, or gangrene threaten amputation. By accessing the blockage from the foot and working upward, our team can often reach and treat below-the-knee and below-the-ankle blockages that are difficult or impossible to treat from a groin approach alone.
